OPT Location of Employment Report Form

DIRECTIONS:

This form is to be completed by F-1 students on OPT.

F-1 OPT regulations require you to report employment information including specific address when you start a new job. Please note that if you change employment you will need to submit this form again. 

* Please remember that if your address changes during your time on OPT you must also complete the Change of Address form.
