Voluntary Life, AD&D and LTD Insurance
The Standard庐 currently provides CSU Employer-Paid Life, Accident Death & Dismemberment
(AD&D), and Long-Term Disability Insurance to benefit eligible employee groups per
their Collective Bargaining Unit.
The Standard庐 also offers voluntary plans to eligible employees as an added way of
protecting your income.
- Voluntary Group Life Insurance
- Employees may purchase protection for themselves and eligible family members including
spouse/domestic partner and dependent children.
- New employees qualify for Guarantee Issue coverage if application submitted within
60 days of start of employment. Under the Guarantee Issue provision, employees may
apply (without answering medical questions) for coverage up to:
- $150,000 for themselves;
- $50,000 for their spouse/registered domestic partner
- $20,000 for each dependent child
- Amounts in excess of the Guarantee Issue, up to the maximum limits, may be chosen
and require evidence of insurabilty.
- Coverage elected more than 60 days from date of employment will be subject to evidence
of insurability.

- Voluntary Group AD&D Insurance
- The plan pays a benefit to the employee or his/her selected beneficiary in the event
of death due to an accident or dismemberment at a benefit level selected by the employee.
- Coverage for spouse, registered domestic partner and dependent child is available
at an additional cost.
- Eligible employees can enroll at any time during the year and will not be subject
to Evidence of Insurability.

- Voluntary Group Long-Term Disability Insurance
- The plan provides loss of income protection (up to 60%) due to a qualified disability.
- New employees may enroll in this plan with Guaranteed Issue (no health questions asked)
within 60 day of employment. Employees with CSU Employer-Paid LTD are not eligible to enroll in a voluntary plan.
- Coverage elected more than 60 days from date of employment will be subject to evidence
of insurability.
- You can purchase a level of insurance that fits your need, with either a 30-day or
90-day waiting period.
